Empowering Independent Living for Adults with Disabilities

In-Home Care for Adults with Disabilities

This service is designed to meet the needs of adults living in the community receiving supports funded through the local Community Mental Health (CMH )or Area Agency on Aging (AAA). This service is more encompassing than the Home Help Program.

After completion of an elaborate assessment of an adult with a disability, CMH may approve funding for the individual to have in-home help for a certain amount of hours per year. The assessment looks at a more complex venue of daily living topics than the areas that the DHS Home Help Program reviews. Typically, a client who needs more help and supervision than what the DHS Home Help Program can provide will apply for additional direct care supports through the CMH or AAA Community Living Supports Program.

Upon authorization from CMH or AAA, staff will provide daily living supports in all aspects of the clients life including: daily chores & routines, bathing, grooming, dressing, transferring, medication, housework, laundry, meal prep, clinical programs, toileting, eating, community activities, skill building (such as socialization, behavioral, daily living tasks, health & safety, community inclusion, etcetera), medical needs such as medical & dental appointments and basic daily medical care duties, financial management, shopping, organizing, and general supervision.

Real Life Living Services can provide these same types of services on a private pay basis for those individuals who do not qualify for CMH or AAA funding.

What are Community Living Support Services

Community Support Living Services are designed to help adults with disabilities live independently while receiving essential support at home and in the community. These services are often funded through Community Mental Health (CMH) or the Area Agency on Aging (AAA) and are more comprehensive than the traditional DHS Home Help Program.

After a detailed assessment, eligible individuals may receive authorized hours of support each year. Our trained staff provide assistance with:

  • Daily Living Tasks – bathing, grooming, dressing, toileting, eating, transferring
  • Household Support – meal preparation, light cleaning, laundry, shopping, organizing
  • Health & Medical Needs – medication reminders, basic medical care, transportation to appointments
  • Skill Building – support with social skills, behavioral health, safety awareness, and personal goal achievement
  • Community Inclusion – participation in community events and meaningful activities
  • General Supervision & Companionship – promoting safety, comfort, and quality of life

If you don’t qualify for CMH or AAA funding, Real Life Living Services also offers private pay options for the same level of personalized support and care.
